There appear to be restored breaks running from the centre of the relief at the top through to the sides approximately a quarter of the way down. There appears to be another restored break running across the bottom quarter of the relief and visible above the crib. There also appear to be restorations to the background of the relief. The right break runs across the praying figure's chest. There are yellowish translucent areas to the mother's breasts. There are various minor chips and scratches to the relief, particularly at the high points of the drapery; the seated figure at the bottom has a lost attribute, and there is a chip to the side of the shield. Otherwise the condition is good, with minor dirt and wear to the surface consistent with age. There are remnants of original polychromy.

The subject of the birth of Saint John is seldom represented in Nottingham alabaster carving, with Cheetham recording only five known examples, including the present, very beautiful, relief (op. cit.). Note the manner in which a scene of private domesticity has been created, with parted curtains revealing the bedchamber.
